  • Annual Permission form/Child Release Permit/Self Driving Certification/Needs Assessment Form can be found here. GREAT ALL IN ONE FORM! Troop Leaders maintain for their personal files.

  • Troop Group Activity Applications can be found here. TGAA is not needed for troop trips that are not high risk, within the state of NJ, or for Council-sponsored events

  • Troop Tax form can be found here. Use when purchasing taxable supplies - the sales tax will be waived since it is for the purposes of a non profit organization.

  • Money Earning form can be found here . This form is used by Troops/Service Units that want to hold money-earning activities in addition to Council-sponsored Fall & Cookie Programs. This form cannot be used for GS Gold or Silver Award candidates. Troops must have participated in both the Fall & Cookie Programs to be approved. GS Daisies do not participate in money-earning activities. Troops must complete the MEA & forward to their Service Unit Manager two months prior to the proposed activity. Adults managing funds must be registered and have completed a criminal background check.

  • Certificate of Insurance Request can be found here. Certificates of Insurance (COI) are issued upon request to companies/organizations providing the use of their
    site to Girl Scout troops/groups for Girl Scout Activities. Many organizations/companies require a COI before
    they allow a group to reserve their site. Please check out this listing of COI’s already on file for GSNNJ     here.
